Job Description

DESCRIPTION OF DUTIES & RESPONSIBILITIES:
• Establishes linkage agreements with social service programs within Northern Manhattan and the Bronx that serve pregnant and post-partum women.
• Delivers educational presentations on chemical dependency and mental health problems and continuum of care for social service program staff from identified social service programs within Northern Manhattan and the Bronx.
• Networks with social service programs for effective coordination of outreach services.
• Delivers educational presentations on chemical dependency problems and available services to pregnant and post-partum women at identified social service programs.
• Conducts chemical dependency and mental health screening for identified clients.
• Coordinates referrals for identified clients to appropriate mental health and chemical dependency treatment programs.
• Interfaces with mental health and substance abuse treatment providers to assure referrals are appropriate.
• Conducts follow up with mental health and substance abuse treatment providers to determine outcomes of referrals for services.
• Ensures referral source satisfaction by addressing any concerns surrounding the outreach process.
• Acts as liaison to local recovery centers and recovery oriented systems of care
• Identifies, interviews and recruits peer mentors
• Maintains a cadre of ten peer mentors
• Schedules and oversees peer mentoring activities
• Provides regular group supervision to peer mentors
• Assures development of personal recovery plan for each woman served
• Maintains documentation of clinical records in accordance with federal, state and local regulations and organizational standards.
• Drives van to pick up/drop off family members for Strengthening Families sessions
• Participates in training as requested by supervisor.
• Prepares statistical reports in order to monitor effectiveness of outreach efforts.
• Performs other duties as required.

Qualifications for position:
Two years of direct service experience in treatment of chemical dependency and mental health problems; One year of direct service experience in treatment of chemical dependency and mental health problems among pregnant and post-partum women, must hold a New York State CASAC credential. Must pass toxicology testing and obtain clearance from NYS Justice Center and NYS Central Register for Child Abuse and Neglect

Supervisory relationships: Senior Family Social Worker

Skills and knowledge required: Knowledge of local social services which serve pregnant and post-partum women, Knowledge of continuum of care for substance abuse and mental health treatment, Knowledge of issues of culture and ethnicity which impact service delivery for African American and Latin women and families, Competent in use of identified screening instruments and protocols, Good interpersonal skills, Good platform skills, Fluent in Spanish

PERSONAL QUALITIES: Well organized, communicates well, able to multi-task.

AMOUNT OF TRAVEL/OTHER SPECIAL CONDITIONS OR REQUIREMENTS: Frequent local and possible out of state travel required, Valid NYS Driver’s license required.
