Job Description

Responsible for providing care to a designated caseload, including provision of direct individual and group therapy services, coordination of specialized program services and case management of services provided by referral.


1. Gather relevant assessment information for the comprehensive evaluation from collateral sources such as referral sources, medical providers, mental health providers, prior chemical dependency treatment providers and the criminal justice system.
2. Participates in the formulation of the comprehensive evaluation.
3. Documents, implements, reviews and updates the comprehensive treatment plan.
4. Writes weekly progress notes.
5. Develop reviews and updates the continuing care plan.
6. Participates in quality management activities as directed by program director.
7. Delivers individual counseling using cognitive behavioral, motivational enhancement therapies and countering the effects of “soldiering.”
8. Facilitates group counseling via Seeking Safety evidence-based trauma intervention.
9. Facilitates anger management, relapse prevention groups using SPECTRUM intervention.
10. Responsible for making appointments with community-based service providers prior to discharge.
11. Plans and facilitates recreational services.
12. Participates as a member of the multi-disciplinary and multi-agency teams.
13. Develops and maintain relationships with all referral sources by providing progress notes and other necessary reporting.
14. Maintains clinical records in accordance with organizational and regulatory guidelines.
15. Participate in in-service training as scheduled to improve overall performance of staff.
16. Performs other duties as may be required.


1. Requires NYSED issued license or CASAC credential recognized as a Qualified Health Professional by funder.
2. Minimum High School Diploma required.
3. Three years of experience in provision of chemical dependency treatment services, one year of experience providing services to a special population such as parolees.
4. Experience in providing cognitive behavioral and motivational enhancement therapies.
